INITIAL VITAL SIGNS: Reviewed by me GENERAL: Awake, well appearing, non toxic HEAD: Atraumatic EYES: No discharge ENT: Moist mucous membranes RESPIRATORY: No respiratory distress. No accessory muscle use. EXTREMITIES: No deformity, ecchymosis SKIN: Warm and dry NEUROLOGIC: Normal mental status and speech. Face is symmetric.
