INITIAL VITAL SIGNS: Reviewed by me GENERAL: Altered, responsive to touch HEAD: Atraumatic EYES:PERRL. No discharge ENT: Dry mucous membranes NECK: Supple RESPIRATORY: Clear to auscultation bilaterally. CV: Regular rate and rhythm. ABDOMEN: no distention per visual inspection EXTREMITIES: No edema. No deformity SKIN: Warm and dry. No rash or petechiae. NEUROLOGIC: Altered, not able to obtain a full neurologic exam
