Eyes: PERL, EOMI, no APD, intact accomodation Face: symmetric facial movement/smile/frown/wrinkle forehead/puffing cheeks, midline tongue protrusion, no facial droop, intact bilateral upper and lower facial sensation Motor: right upper extremity 5/5, left upper extremity 5/5, right lower extremity 5/5, left lower extremity 5/5, no perceptible weakness Sensation: intact sensation throughout the body/extremities No pronator drift, intact finger to nose, normal gait, normal heel to toe walking
